How do I reach CMI with questions about my account?

Where do I send payment?

To ensure proper processing, please include the patient's full name and account number on all payments. We accept checks, money orders, or major credit card numbers with expiration date and your signature.

Do you accept payment plans?

Yes. Please call (503) 292-9108 & a business office representative will be happy to discuss payment plans.

What do I need to bring with me to facilitate the payment process?

Bring your complete insurance information, including your insurance card, ID number, and group number. In addition, for workers' compensations claims and automobile injuries, bring your claim number & date of injury.

Do I need preauthorization from my heath-care policy or plan?

Some plans do require preauthorization. Check with yours to see if this is necessary. Call the number generally located on the back of your insurance card. Your insurance company can also answer any questions you may have about what is covered and what is not.

Does CMI file secondary claims as well as primary ones?

Yes, we do.

I don't have medical insurance. Can I get a price quotation on my procedure?

If you do not have medical insurance and/or you are concerned about paying for your procedure, please call the CMI business office at (503) 292-9108. We'll give you an estimate and work with you in setting up a payment plan.
