Privacy Practices

Introduction

Most of us feel that our health and medical information is private and should be protected, and we want to know who has this information. Federal law:

  • Gives you privacy rights over your health information.
  • Sets rules and limits on who can look at and receive your health information.

These rights are important for you to know. You can exercise these rights, ask questions about them, and file a complaint if you think your rights are being denied or your health information isn't being protected.

Our Joint Privacy Notice includes two separate sections:

Patients' Rights Regarding Protected Health Information (PHI) describes your rights to access and control your health-care information and how you can exercise your rights. We recognize that health-care information about you is personal. We are committed to protecting the confidentiality of your health-care information

Permitted Use and Disclosure of Your Health-Care Information describes how The Center for Medical Imaging (CMI) may use and disclose your health-care information to carry out treatment, payment, or health-care operations and for other purposes that are permitted or required by law. This notice covers only the health-care information collected, created, and maintained by, through, or at CMI.

This notice applies to all facilities, services, and programs provided to you. Our medical staff, employees, volunteers, students, trainees, and all other members of the CMI workforce will comply with this notice. When we refer to "CMI" in this notice, we include all those named in this paragraph.We are required by law to maintain the privacy of your medical information.
